Item Functional recovery of upper limb post-stroke : mental practice with motor and non-motor imagery.(2012) Bastos, Aline Furtado; Carrapatoso, Beatriz Cantanhede; Orsini, Marco; Leite, Marco Antonio Araujo; Silva, Julio Guilherme da; Souza, Gabriela Guerra Leal deProblem statement: Mental Practice (MP) with motor imagery consists of a method of training in which motor imagery is used to improve performance. The aim of this study was to investigate the efficacy of MP with motor imagery on motor recovery of the paretic upper limb of poststroke patients in comparison with MP with non-motor imagery. Approach: Four post-stroke patients were divided into two groups: experimental (the physical practice of motor tasks and MP of the same tasks: n = 2) and control (the physical practice of motor tasks and MP of non-motor tasks: n = 2). We evaluated: Heart Rate (HR) and systolic (SBP) and Diastolic Blood Pressure (DBP), immediately before and after execution and imagination of the tasks; time of execution and imagination of each task; and motor function of the paretic upper limb through the questionnaires “Fugl-Meyer for Upper Extremity” and “quality of movement section of arm motor ability test” before and after the 10 weeks of treatment. Results: The qualitative analysis showed that regardless of training, patients spent less time on imagery motor tasks than to execute them; and have increased in HR, SBP and DBP after execution as well as after imagery of motor tasks. The training caused a reduction in the time of execution of motor tasks in both groups and increment on the motor function of the paretic upper limb only in the experimental group. Conclusion: These results suggest that an MP with motor imagery may be an effective method for the functional recovery of the paretic upper limb of post-stroke patients.Item Physical therapy improved hand function in a patient with traumatic peripheral lesion : a case study.(2012) Orsini, Marco; Silva, Julio Guilherme da; Correa, Clynton Lourenco; Rodrigues, Diego; Oliveira, Acary Bulle; Coelho, Valéria Marques; Gollo, Debora; Catharino, Antonio Marcos da Silva; Machado, Dionis; Bastos, Victor Hugo do Vale; Leite, Marco Antonio Araujo; Souza, Gabriela Guerra Leal de; Reis, Carlos Henrique Melo; Menezes, Sara Lúcia Silveira deProblem statement: Nerves are frequently injured by traumatic lesions, such as crushing, compression (entrapment), stretching, partial and total extraction, resulting in damages to the transmission of nerve impulses and to the reduction or loss of sensitivity, to the motility and to the reflexes of the innervated area. The objective of this study was to evaluate the results of a rehabilitation program that lasted three months in the process of traumatic injury recovery of the median and ulnar nerves in a 52 year-old patient. Approach: The patient underwent an evaluation of the muscle strength and the functional capacity before and after three months of rehabilitation treatment, consisting of movements and specific techniques of Proprioceptive Neuromuscular Facilitation (PNF), which lasted approximately 50-60 min per session and with a frequency of three weekly visits. She also performed stretching, neural mobilization maneuvers, ultrasound and laser. Results: The physical therapy approach proposed, in the present case, minimized the injury impact and facilitated the gradual return of patients to basic and instrumental activities of daily living. It was possible to observe improvement on the functional abilities and the muscle strength after the end of the protocol. Conclusion: The morphological and functional recovery after a nerve injury is rarely complete and perfect, as it was in this case. However, a proper clinical management, combined with a rehabilitation protocol can minimize the deficiencies and facilitate the return of patients to their daily activities.
