Item Associação de fatores sociais e biológicos com transtornos de ansiedade e depressão : ênfase na insegurança alimentar e no metabolismo da vitamina D.(2024) Sabião, Thaís da Silva; Carraro, Júlia Cristina Cardoso; Coelho, George Luiz Lins Machado; Coelho, George Luiz Lins Machado; Ferreira, Carolina Nicoletti; Pimenta, Adriano Marçal; Aguiar, Aline Silva de; Souza, Anelise Andrade deEste estudo teve como objetivo analisar os fatores associados aos transtornos de ansiedade e depressão, com foco na insegurança alimentar e no metabolismo da vitamina D. Foram empregados dados de um inquérito de base populacional no Brasil, e de uma coorte espanhola. Para o estudo transversal, foram utilizados dados do inquérito COVID- Inconfidentes, realizado entre outubro e dezembro de 2020 em duas cidades de Minas Gerais, Brasil. Por meio de entrevista face a face, foram avaliadas as variáveis socioeconômicas, de estilo de vida e condições de saúde. Foram coletadas amostras de sangue para análises bioquímicas e genéticas. As variáveis desfecho foram os sintomas do transtorno de ansiedade generalizada (TAG) e os sintomas de transtorno depressivo maior (TDM), avaliados pela Generalized Anxiety Disorder-7 e pelo Patient Health Questionnaire-9, respectivamente. Foram avaliados: 1) a insegurança alimentar (exposição), utilizando a Escala Brasileira de Insegurança Alimentar, e sua associação com desfechos, comparando as diferenças sociodemográficas; 2) os níveis séricos de vitamina D (exposição) e sua associação com desfechos, definindo deficiência como <20 ng/mL para a população saudável ou <30 ng/mL em grupos de risco; analisando a influência do polimorfismo do gene VDR isoladamente (como variável de interação) e de um escore de risco genético (como variável de estratificação e de interação) que combinou os genótipos dos polimorfismos dos genes DHCR7 (rs11606033), GC (rs7041) e VDR (rs2228570). Para o estudo longitudinal, foram utilizados dados da coorte Seguimiento Universidad de Navarra (Estudo SUN), que acompanha graduados universitários espanhóis a cada 2 anos desde 1999, e permanece aberta continuamente. Questionários são enviados aos participantes por e-mail ou correio, avaliando hábitos, comportamentos e condições de saúde. Foi investigada a associação da vitamina D (exposição) com o risco de depressão, considerado o desfecho do estudo, analisando-se a influência de características individuais e hábitos de vida. Os participantes inicialmente livres de depressão foram classificados como casos incidentes se relataram um diagnóstico médico de depressão durante o acompanhamento. Os níveis séricos de vitamina D foram calculados usando uma equação validada, e a deficiência foi definida como níveis abaixo de 20 ng/mL. No estudo transversal, a prevalência de sintomas de TAG e TDM foi de 23,5% e 15,8%, respectivamente. Foi observada uma associação direta e proporcional entre a gravidade da insegurança alimentar e os sintomas de TAG e TDM. Indivíduos em insegurança alimentar grave apresentaram uma prevalência 3,56 vezes maior de sintomas de TAG e 3,03 vezes maior de TDM. Foram observados piores resultados para indivíduos de cor da pele não branca, sem filhos e com menor renda familiar mensal. Em relação à vitamina D, não foi observada associação direta com os sintomas de transtornos mentais. No entanto, a análise de interação revelou um efeito combinado da deficiência de vitamina D e do polimorfismo do gene VDR nos sintomas de TDM, mas não em TAG. Indivíduos com deficiência de vitamina D e com um alelo alterado apresentaram uma prevalência 2,84 vezes maior de TDM, enquanto aqueles com a presença de dois alelos alterados tiveram uma prevalência 4,37 vezes maior de TDM. Ao estratificar os participantes pelos tercis do escore de polimorfismos, observou-se uma associação exclusiva entre deficiência de vitamina D e sintomas de TAG e TDM apenas para os indivíduos do tercil mais alto desse escore (maior número de alelos de risco para deficiência de vitamina D). Na análise de interação, os indivíduos com deficiência de vitamina D do segundo tercil do escore apresentaram uma prevalência 2,32 vezes maior de TDM, enquanto os com deficiência de vitamina D do tercil mais alto apresentaram uma prevalência 3,79 vezes maior. Houve interação entre fatores genéticos e deficiência de vitamina D para TDM, mas não para TAG, e foi exclusiva entre os que tinham exposição solar insuficiente e não suplementavam vitamina D. No estudo longitudinal, entre 192.976 pessoas-anos de acompanhamento, foram identificados 753 casos incidentes de depressão. Participantes com deficiência de vitamina D apresentaram um risco 27% maior de depressão. Além disso, foi observada uma interação com o sexo feminino. Em conclusão, houve uma forte associação entre insegurança alimentar e sintomas de TAG e TDM, influenciada pelas características sociodemográficas. Embora inicialmente não tenha sido encontrada uma ligação direta entre deficiência de vitamina D e os sintomas de transtornos mentais, análises adicionais destacaram sua interação complexa com polimorfismos de genes do metabolismo da vitamina D e a influência de hábitos de vida. Além disso, os resultados sugerem uma relação causal entre deficiência de vitamina D e o risco de depressão, especialmente entre mulheres.Item Atividade física durante a pandemia de Covid-19 : interfaces com a saúde física, qualidade do sono e autoavaliação de saúde.(2024) Moura, Samara Silva de; Meireles, Adriana Lúcia; Carraro, Júlia Cristina Cardoso; Meireles, Adriana Lúcia; Sales, Aline Dayrell Ferreira; Parajára, Magda do Carmo; Oliveira, Lenice Kappes Becker; Menezes Júnior, Luiz Antônio Alves deIntrodução: As evidências científicas produzidas em relação à atividade física (AF) são consideráveis e o aumento exponencial nos últimos anos forneceu informações congruentes no que concerne à sua importância e benefícios à saúde. Uma rotina regular de AF tem sido associada a uma melhor saúde física, como controle da homeostase glicídica e lipídica, bem como com uma melhor qualidade do sono e autoavaliação de saúde (AAS). Contudo, a pandemia da Covid-19 influenciou drasticamente a prática de AF. Objetivo: Avaliar os fatores associados à inatividade física (IF em residentes da região dos Inconfidentes, Minas Gerais, durante a pandemia da Covid-19. Métodos: Estudo transversal de base populacional em adultos, realizado de outubro a dezembro de 2020, em dois municípios brasileiros (Ouro Preto e Mariana). Foram coletados 1762 indivíduos, por meio de entrevista face a face, utilizando um questionário estruturado em formato eletrônico com questões sociodemográficas, hábitos de vida e condições de saúde. Foram conduzidas coletas de sangue para avaliação dos níveis séricos de lipoproteínas e concentração plasmática de hemoglobina glicada. A prática de AF foi avaliada por autorrelato, por questão adaptada do estudo "Vigilância de fatores de risco e proteção para doenças crônicas por inquérito telefônico “(VIGITEL)". A AF foi classificada como fisicamente ativos ou inativos, de acordo com as recomendações da Organização Mundial da Saúde (OMS). A qualidade do sono foi medida pelo Índice de Qualidade do Sono de Pittsburgh (IQSP). A AAS foi avaliada por meio da pergunta: “Como você classificaria seu estado de saúde?” As respostas foram agrupadas em AAS negativa (regular, ruim ou muito ruim) e AAS positiva (muito bom ou bom). Além disso, foram mensurados os níveis séricos de lipoproteína de alta densidade (HDL-c) e o colesterol total (CT), usando o método enzimático colorimétrico. As concentrações plasmáticas de hemoglobina glicada (HbA1c) foram avaliadas por meio do método imunoturbidimetria. Resultados: Os resultados serão apresentados em cinco artigos. No artigo I observou-se que a prevalência da IF foi mais elevada nos primeiros meses (março a julho/2020) da pandemia (M1) (67,3%), em comparação com os meses de outubro a dezembro de 2020 (M2) (58,7%) e no período anterior ao início da pandemia (M0) (39,7%). Indivíduos com sobrepeso, obesidade e baixo nível de escolaridade apresentaram maior probabilidade de serem fisicamente inativos. No artigo II, 51,9% apresentavam má qualidade de sono e 79,8% não estavam em trabalho a partir de casa (home office). A IF associou-se à má qualidade do sono (OR: 1,73; IC95%: 1,07-2,79) e ausência de home office (OR: 1,86; IC95%: 1,08-3,23). Na análise combinada, os indivíduos com qualidade de sono ruim e que não estavam em home office tinham uma chance 4,19 vezes maior de serem inativos fisicamente (OR: 4,19; IC 95%: 1,95-8,95). Referente ao artigo III, 7,1% dos participantes apresentaram níveis elevados de hemoglobina glicada (HbA1c). A análise de mediação revelou que a IF aumentava em 2,62 vezes (OR:2,62, IC95%:1,29-5,33) a probabilidade de níveis elevados de HbA1c, sendo que 26,87% desse efeito foi mediado pelo excesso de peso (OR:1,30; IC95%:1,06-1,57). O artigo IV apresentou prevalência de risco cardiovascular (RCV) intermediário a alto de 43,8% dos participantes e 56,0% estavam com excesso de peso. Na análise de mediação para o excesso de peso, constatou-se que os indivíduos fisicamente inativos apresentavam 1,90 vezes mais chance (OR: 1,90; IC95%; 1,14-3,17) de ter RCV intermediário a alto, sendo que 17,18% desse efeito foi mediado pelo excesso de peso (OR: 1,11; IC95%; 1,02-1,25). No artigo V, a prevalência de AAS negativa foi de 23,0% dos participantes e 15,5% tinham comportamento sedentário (CS) ≥ 9 horas. O modelo multivariado identificou associações entre IF e AAS negativa (OR: 1,72; IC95%: 1,01-2,93), mas não para CS. A análise de simultaneidade revelou que os indivíduos afetados por um destes comportamentos de risco à saúde tinham 1,84 vezes mais probabilidade de apresentar AAS negativa (OR: 1,84; IC95%: 1,06-3,22). Aqueles com dois comportamentos de risco à saúde tinham 2,12 vezes mais probabilidade de ter AAS negativa (OR: 2,12; IC95%: 1,03-4,39). Na análise conjunta, os que relataram ser fisicamente inativos e apresentarem CS ≥ 9 horas tiveram uma prevalência 2,15 vezes maior de AAS negativa (OR: 2,15; IC95%: 1,01-4,56). Conclusão: Este estudo revelou uma elevada prevalência de IF durante a pandemia da Covid-19. Além disso, evidenciou associação entre a IF e marcadores bioquímicos, como alterações nos níveis de hemoglobina glicada, e maior prevalência de RCV. Adicionalmente, a IF e o CS, em análise conjunta e simultânea mostraram associação com a AAS negativa. Por fim, a má qualidade do sono e não estar em home office foram associados a IF. Este estudo ressalta a importância de um comprometimento efetivo por parte dos fomentadores de políticas públicas, governantes, profissionais de saúde e pesquisadores para estabelecer medidas que incentivem e facilitem a adoção de estilos de vida mais ativos, com benefícios inquestionáveis para a saúde e o bem- estar da população.
